Unified Healthcare of Idaho accepts Medicare, Medicaid, Commercial Insurance, Private Pay and Bishop Pay for our medical expense coverage. We also offer a Sliding Fee Discount Program for those who apply. We will not turn away any patient regardless of their financial situation. Notice To Patients-Sliding Fee Discount Program
Unified Healthcare of Idaho accepts all patients regardless of their ability to pay. Discounts for essential services are offered based on family size and and income. For more information, contact our office, or download the Sliding Fee Discount Application below. Fill out the medical paperwork form and return it to one of our receptionists to determine if you or your family members are eligible for a discount. We know that your financial situation can change suddenly and without warning. For our patients who do not have insurance, do not have enough insurance, or if your family is just going through a difficult time, we provide relief. In order to apply please fill out the medical paperwork found below. This medical expense coverage form is to be completed every 12 months or if your financial situation changes. The discount will apply only to services received at this clinic.
New Patient Forms
New patients with Unified Health Care of Idaho save time during their first appointment by completing their new patient forms at their convenience. Below please find all applicable forms needed for for both adult and child new patients. You can download and fill out new patient forms before your first appointment or you can do it at the office, if you’d prefer.
